Preparing for Your Visit
Thank you for scheduling an appointment with our office online. This page is to provide you with important information to help you be well-informed for your visit.
For Dermatology visits, prior medical records are generally not necessary unless specifically asked for by the doctor.
Please arrive 10 minutes prior to your appointment time.
If you are new to our practice, the following forms can be printed and filled out prior to your visit. By filling out these forms ahead of time, you will save a significant amount of time during your first office visit.
Insurance Information
We are In-Network with Medicare and many PPOs, but Out-of-Network with most Narrow PPOs. Nobody likes financial surprises, so we recommend you review the details below. Or feel free to call our office with any insurance questions at (949) 364-8411.
It is our recommendation that patients check their insurance directory online with their login or call their insurance about a week before the visit to confirm that your doctor is in your network, as well as what specific benefits will apply.
We ask our patients to bring their insurance card to every visit so that we can help patients stay informed of their insurance status with our office.

Appointment Reminders and Cancellations
Look for an appointment reminder via email and a Self Check-In link via SMS. Please contact us if you cannot make your appointment time so we can offer it to another patient.
