Our office hours are Monday –Thursday from 8am to 5pm and Friday from
8am-3pm. If you have an emergency after hours or on weekends, please call to be forwarded directly to Dr. Lener, Dr. Litchfield, Dr. Khoury. or Dr. Goodarzi
Physicians at The Dermatology Center at Ladera accept appointments for patients regardless of their insurance status. We participate In-Network with most PPO plans, Medicare, and Tricare. Patients with HMO insurance, Medi-Cal/Cal-Optima, Healthy Families, or without insurance will be asked to pay our office directly for their care on the date of service, and will receive a 20% discount. We have set our fees fairly so that patients who value our high level of care can be treated at reasonable rates.

For patients who are covered under of one of the PPO plans in which we are In Network, we will ask for any co-pay in advance. In some exceptional cases, we will also ask for coinsurance or deductible prior to treatment.
Out of Network PPO Provider:
- United Healthcare/Pacificare PPO
- CalPERS PERS Select
Out of Network PPO patients will be asked to pay their co-pay at their visit, and we will submit the claim to their insurance. After insurance processes the claim, we will send the patient a statement for the remaining balance not paid by co-pay and their insurance. It is our recommendation that patients check with their insurance company about benefits for out of network providers, so that there are no financial surprises. Sometimes there is a separate deductible or other payment differences. We have many patients that are cared for on an out of network basis and are pleased with how it is handled at our practice.
Our office does not participate with, accept, or do any administration or documentation for worker’s compensation claims.

If you have any questions when filling out the form, please do the best you can, our staff will assist you with your questions on the day of your visit. Please fax your form back to us at 949.364.8511 or arrive 10 minutes early with your completed form.
